Required Documents (forms must be complete and include appropriate signatures):
  • Participant Application / Parental Consent
  • Medical Authorization
  • Code of Ethics
  • Parent Handbook acknowledgment (from last page of handbook)
  • Proof of age (new participants only)
If you register online you do not need to submit the Participant Application. You will have given your consent when you registered your child(ren). 

You may substitute the Medical Clearance with a form provided by your child's Physician. 

Acceptable Proof of Age: 

A legible copy of a county certified or notarized birth certificate, military dependent ID, passport or adoption papers.
